How Our Crisis Intervention Works
One on One Meetings
Our Crisis Navigator meets with you personally during intake. Well discuss your needs and current situation in a safe confidential environment. No judgment, just understanding and practical help.
Customized Care Plan
Together with our Case Management Team we create a personalized care plan that’s unique to you. This plan outlines specific programs and services that match your individual needs. We’re not offering one size fits all solutions.
Long Term Roadmap
Your detailed case plan provides step by step guidance. It includes both immediate and long term goals to stabilize your life. Our aim is helping you achieve economic self sufficiency and lasting stability.

Our dedicated staff connects you with community resources and emergency support to overcome immediate needs. But we don’t stop there, we help you create a long-term plan for self-sufficiency to prevent similar problems in the future.
Your Journey to Stability Starts Here
Each participant begins with a formal intake process. You’ll meet one on one with a Case Management Team Member who listens to your story and understands your situation. Together you’ll create a customized care plan designed specifically for your needs.
Your care plan includes achievable action steps that guide you forward. Need to meet with a substance abuse counselor? We arrange it. Need to get a drivers license? We help make it happen. Along the way participants receive practical support like food vouchers bus passes or gas cards as determined by their Case Manager.

Crisis Navigation gives you the opportunity to reach your goals one step at a time. Each completed step on your personal care plan brings you closer to stability. Whether its obtaining vital documents finding employment or accessing mental health services were here to support your progress.
